New Patients

Prior to your initial visit at the Clinic, you must set up a new patient chart to register yourself and show valid proof of enrollment and eligibility. The Medical Records staff will assist you with the information you need in order to establish your chart. The following items are needed to verify eligibility for services inside the Clinic and for outside referrals:

Tribal ID

Driver’s license or other federal or state ID

Proof of permanent address

Birth certificate

Charts are created at the clinic BY APPOINTMENT ONLY.

WHAT TO EXPECT AT YOUR FIRST APPOINTMENT

Once you have visited Medical Records and established eligibility, an appointment may be made through the scheduling department. Here is what to expect when you come to the health clinic for your initial appointment:

Please arrive for your first appointment at least fifteen minutes early.

You will fill out other new patient paperwork.

You must bring your private insurance, Medicare, or Medicaid cards and a photo ID with you each time you visit.

You will be asked to verify your registration information at each appointment and make relevant updates. The Clinic accepts and participates with most insurance plans.
