On-site services are available to any enrolled member of a federally recognized Tribe. A Tribal Member must be able to show documented proof of enrollment. Services are also provided to First Generation Indian Descendants of the Poarch Creek Indians, from birth to the age of nineteen. Patients are required to be seen yearly in the health clinic to keep their health histories and medical record updated.
Prior to your initial visit at the Clinic, you must set up a new patient chart to register yourself and show valid proof of enrollment and eligibility. The Medical Records staff will assist you with the information you need in order to establish your chart. The following items are needed to verify eligibility for services inside the Clinic and for outside referrals:
Once you have visited Medical Records and established eligibility, an appointment may be made through the scheduling department. Here is what to expect when you come to the health clinic for your initial appointment:
You will be asked to verify your registration information at each appointment and make relevant updates. The Clinic accepts and participates with most insurance plans.
Enrolled Poarch Creek Tribal Citizens living within the six county service area that currently utilize the Buford L. Rolin Health Clinic may be eligible for referrals for specialty care through the Purchase and Referred Care program (PRC), formally known as the Contract Health Service (CHS) program. If you need assistance with finding or applying for insurance, our Resource Coordinator (extension 2324) will be happy to help you find the appropriate coverage for your needs. A patient must reside within the six county service area for at least ninety days before they are considered eligible for Purchased and Referred Care services. When moving in or out of the service area, patients must update their information with the clinic.

Patient must present two proofs of residency such as:
• Current electric bill
• Landline phone bill
• Mortgage statement, or
• Rental agreement
• Please arrive for all appointments at least five minutes early to allow time to update records and obtain necessary information.
• All patients that arrive fifteen minutes late for appointments will be asked to reschedule or be a “work in” due to time constraints.
• Please call the health clinic in advance if you have a special circumstance that prevents you from making your scheduled appointment time.
• Provide accurate and complete medical information to the best of your ability.
• Communicate with the clinical staff if you have questions or concerns about your course of medical care.Follow the treatment plan recommended by your physician. After all, they are here for your well-being.
• Be considerate of other patients and follow Clinic guidelines.
• Bring insurance cards and photo ID to every appointment.
• Write down any questions or concerns for the medical provider.
• Bring a list of current medications.
• Children under eighteen must be accompanied by their legal guardian or have a current consent on file for another caregiver.
We strive to keep wait times to a minimum, but unfortunately delays can occur due to unforeseen patient needs. If you wait more than fifteen minutes, please remind the registration staff that you are waiting.
