COMMUNITY HEALTH DEPARTMENT


The Community Health Department assists with health care needs of patients and their families within the community.

Nurses provide assessment of need for patient education and intervention for a variety of issues, usually in the patient’s home or other residential setting, or community health premises. The groups of people they serve include people with identifiable physical conditions or mental illness or disability, or people whose poor health mandates support services so they can continue to live in the community.


Types of Community health services include, but are not limited to:

Health/Medication education and management

Preventive Services

Intervention or health promotion provided as a matter of public policy

immunization Promotion

Nursing assessment of health status and health needs

Home safety assessments

Coordination services with home health, hospice, medical providers, family services, housing, and tribal citizen benefits as needed

HEALTH EDUCATION INCLUDES, BUT IS NOT LIMITED TO:

Annual Health Fair

Youth Health Camp

Wellness Wednesday Screenings

Car Seat Safety Program

Health Awareness Walks

CPR Classes

Community Lunch and Learns

Smoking Cessation

Health Education provides services and hosts events in order to promote and educate our community and staff to lead a healthier lifestyle.
