COMMUNITY HEALTH DEPARTMENT
The Community Health Department assists with health care needs of patients and their families within the community.
Nurses provide assessment of need for patient education and intervention for a variety of issues, usually in the patient’s home or other residential setting, or community health premises. The groups of people they serve include people with identifiable physical conditions or mental illness or disability, or people whose poor health mandates support services so they can continue to live in the community.
