Communication and coordination are at the center of the PCI Health Department’s Integrated Care, an innovative model of healthcare delivery that goes beyond mere medical treatment, striving to encompass a holistic view of health that takes into account the physical, mental, and social well-being of our Tribal Citizens.


Meet Your Integrated Care Team

A primary care provider (PCP), often referred to as a primary care physician or general practitioner, is a healthcare professional who serves as a patient’s main point of contact for routine medical care. They play a crucial role in managing an individual’s overall health and well-being.

Your Case Management Support (CMS) plays a vital role in coordinating care, improving outcomes, and ensuring cost-effective healthcare delivery. This support is particularly valuable for individuals with complex medical conditions, chronic diseases, or those who require long-term care.

A case manager is a Registered Nurse (RN) who coordinates treatment plans and prescriptions between primary care physicians within PCI Health Dept and outside providers to ensure that patients receive appropriate and timely healthcare services while optimizing resources and minimizing costs.

A Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) is a vital member of the Integrated Care Team who performs clinical or administrative tasks, checks vital signs, and assists with various screenings and vaccinations.

Healthcare is a complex field with numerous specialists who work together to provide comprehensive care to patients. In addition to the Integrated Care Team, our Tribal Citizens are also supported by Behavioral Health Consultants, Nutritionists and Pharmacists to ensure the best health possible at any stage of life.
